We're delighted to welcome you and your child to CAHA! Our pediatric team is committed to providing compassionate, comprehensive care to help your child grow up healthy and happy. Whether you’re visiting us for the first time or transitioning from another provider, we want to make the process as smooth and stress-free as possible.
What to Expect on Your First Visit
At your child’s first appointment, we’ll take time to get to know your family, review your child’s medical history, and perform a thorough physical exam. We believe in partnering with parents, so we’ll answer all your questions and provide guidance tailored to your child’s age and developmental stage. Please arrive 15 minutes early to allow time for check-in and paperwork.
What to Bring
To help us serve you best, please bring the following to your first visit:
-Your child's insurance card
-Valid photo ID for the parent or guardian
-A list of current medications and allergies and an address of your local pharmacy
-For patients transferring in, we require vaccine records to be sent prior to scheduling your first appointment, and full medical records can be sent anytime before the date of the appointment. You can fax them to 312-943-6924 or email them to us at [email protected].
We do not accept new patients who are not vaccinated or plan to not vaccinate. Click here for our formal vaccine policy
Insurance and Billing
We accept most major insurance plans. Please call our office at 312-943-6964 to confirm if we are in-network with your provider. Copayments and any balances are due at the time of service. Our billing team is happy to answer any questions you may have.
Prenatal appointments
If you are interested in attending a prenatal event or meeting one-on-one with a provider, click HERE. This is not required to become a new patient; all providers in our office are accepting new patients and are happy to see your baby! If you are delivering at Prentice Women's Hospital, delegate any of our doctors as your pediatrician and we will see you at the hospital. You do not need to pre-register with our office.
Parking
Parking is available via the covered, attached garage. The garage entrance is located on North Clark St. south of the office. The parking rate is $6.50 for 3 hours with parking validation (bring your ticket with you into the office for validation). Once you park, you will take the elevators to the main lobby. From the main lobby you will take the second set of elevators to the 4th floor. Additionally, easy metered street parking is available on Clark St. and nearby side streets.
