New Patients

Prenatal

If you are interested in attending a prenatal event or meeting one-on-one with a doctor, click HERE.  This is not required to become a new patient; all doctors in our office are accepting new patients and are happy to see your baby!  If you are delivering at Prentice Women's Hospital, delegate any of our doctors as your pediatrician and we will see you at the hospital.  You do not need to pre-register with our office.

Newborns

Once your baby is born, call our office to schedule the first appointment.  Newborns typically need to be seen 2-4 days after discharge from the hospital, and the doctor in the hospital will help guide you. 

Transfer patients

Please have your child's medical and vaccine records sent to us at least 1 week prior to the first appointment.  You can fax them to 312-943-6924 or ask the front desk for our email address when you call to schedule the appointment.

Please note, we are actively working on a new vaccine policy to better protect our patients. We are no longer accepting new patients who are not vaccinated or plan to not vaccinate. Please stay tuned for updates.

Insurance

The list of insurance plans we accept can be found HERE.  Because insurance is constantly changing, we recommend you double check with your provider that we are in network.
